Diarrhea is considered chronic when it persists for 4 weeks or longer. It is a common but frequently underinvestigated symptom that can significantly affect quality of life and, when untreated, may lead to dehydration, malnutrition, and nutrient deficiencies. A gastroenterologist can identify the underlying cause and develop an effective treatment plan.
Common Causes of Chronic Diarrhea
-
Irritable bowel syndrome (IBS-D) — the most common cause of chronic diarrhea
-
Inflammatory bowel disease (Crohn's disease, ulcerative colitis)
-
Celiac disease and non-celiac gluten sensitivity
-
Small intestinal bacterial overgrowth (SIBO)
-
Microscopic colitis — diagnosed only by colonoscopy with biopsy
-
Bile acid malabsorption — especially after gallbladder removal
-
Lactose intolerance or other food intolerances
-
Infections — Clostridioides difficile (C. diff), parasites
-
Medication side effects — antibiotics, metformin, NSAIDs
When to See a GI Doctor
See a gastroenterologist if diarrhea persists for more than 4 weeks, is accompanied by blood or mucus, causes significant weight loss or fatigue, or disrupts your daily life. Our physicians will perform a systematic evaluation to identify the cause.
Diagnosis
Workup typically begins with blood tests (CBC, metabolic panel, celiac antibodies, thyroid), stool studies (C. diff, infection, fecal calprotectin for IBD), and may include hydrogen breath testing for SIBO, colonoscopy with biopsy (to diagnose microscopic colitis and IBD), and upper endoscopy with small bowel biopsy (to diagnose celiac disease).
